Support Workers

At Hunter Care Group, our focus is on you. We utilise a Case Management model that are able to build supports according to your needs and interests, keeping things personal in an “online world”. We ensure that you receive consistent supports with a worker of your choosing whilst having a point of contact within our Case Management team.

All HCG Support Workers are qualified in the appropriate areas and have the required checks to ensure that your supports are tailored to you!

Coordination of Supports

At Hunter Care Group, we understand the complexities and struggles that participants face with the NDIS. Our experienced Support Coordinators are able to assist you with all of your Support Coordination needs and ensure that you are utilising your plan to its fullest potential.

Our Support Coordinators in Newcastle are trained appropriately and the diverse team ensures that you will be allocated a Coordinator suited to your requirements. Hunter Care Group do not utilise Coordinators on a part-time basis, meaning that they are always contactable and able to assist.

Specialist Coordination

Hunter Care Group have an experienced team of qualified Specialist Coordinators that are tailored to the participant and meet the requirements of the NDIS Commission. Our Specialist team have the knowledge and experience to assist with any crisis or complex needs that a participant may be experiencing.

Our Specialist team also link in with the Coordination team to ensure transparency between both lines of Coordination and ensure that the require communication is maintained throughout your supports.

Case Management Platform

Hunter Care Group is aware of the lack of “quality assurance” in the NDIS market.
To provide the best possible experience for our participants, we have a fully-funded Case Management team. This ensures that every participant we support, has access to their own specifically allocated Case Manager.


We are aware that in the Community Services industry, one missing factor is quality communication. To provide the best communication possible, HCG only employ full-time Managers and Coordinators, ensuring that all lines of communication remain open and accessible to our participants.
